If it is not safe for you to receive emails at your CDU student email address, please complete this form instead of booking through UniHub. Completing this form allows our team to manually book your appointment, which means no automated confirmation or reminder emails will be sent to your CDU student email address.

This form also allows you to tell us how you would like us to contact you. Please let us know your preferred method of communication, any safe contact times, and any other information that will help us communicate with you safely. We will make every effort to follow your contact preferences when arranging and confirming your appointment.

Our team includes practitioners of different genders. If you have a preference for the gender of the practitioner you see, please let us know. We will do our best to accommodate your preference where possible.
